- Photocopy of Birth Certificate (if Single)
- Photocopy of Marriage Certificate (if Married)
- Latest Residence Certificate (Cedula)
- Proof of Billing ( Water Bill, Electric Bill, etc.)
- Photocopy of Valid ID with 3 Specimen Signature (Preferably Gov't ID issued)
- Latest Payslip duly signed by the Signatory
- Updated Certificate of Employment or Contract of Employment Certified or with authorized Signature
- Plane Ticket
- Photocopy of Passport and Visa (Stamp with Arrival and Departure)
- Special Power of Attorney (Certified by Philippine Embassy or Consulate of the Foreign Country)
